Facility Use Request Form

Thank you for your interest in utilizing Cody Stampede Park. This Facility Use Request Form is required for all individuals, organizations, and businesses seeking to host an event, activity, or gathering on Stampede Park grounds. Submission of this form allows the Cody Stampede Board and management to review your request for availability, suitability, and compliance with park policies and operational requirements.

Completion of this form does not guarantee approval. All requests are subject to review, and additional documentation, permits, or approvals may be required depending on the nature and scope of the event.

Insurance Requirements:
Certain events will be required to carry general liability insurance. If required, a certificate of insurance must be provided to the Cody Stampede office prior to the event date, listing Cody Stampede as an additional insured. Failure to provide proof of insurance by the specified deadline may result in denial or cancellation of the event.

Additional Requirements and Considerations:

• A detailed description of the event, including setup, schedule, and breakdown, must be provided.
• Applicants may be required to obtain applicable city, county, or state permits and comply with all local regulations.
• Depending on the event, security, medical services, traffic control, and sanitation plans may be required and must be approved in advance.
• Food and beverage service, including alcohol, must be coordinated with Cody Stampede and comply with all licensing requirements.
• The applicant is responsible for any damages to the facility and must leave the premises in a clean and orderly condition.
• Setup and teardown times must be adhered to as approved.
• The Cody Stampede reserves the right to deny any request that conflicts with scheduled events, operational needs, or organizational policies.

All approved users will be required to sign a formal facility use agreement outlining terms, fees, responsibilities, and conditions of use.
